We are pleased to welcome you and your family to Magee Rehabilitation. Our rehabilitation team will be working with you to
help you achieve your maximum potential and become as independent as possible. We believe your family’s participation is an
essential part of the rehabilitation process. Those most important to you are strongly encouraged to attend individual teaching
sessions, classes and support groups. Familiarizing your family with your care will result in a smooth transition from Magee
to your home.
About Magee
Magee, a founding member of the Jefferson Health System, is the Philadelphia Region’s first rehabilitation hospital, opening
its doors in 1958. Magee is also home to the nation’s first brain injury rehabilitation program to be accredited by CARF,
The Rehabilitation Accreditation Commission, in partnership with MossRehab, Magee is part of a model system for brain injury
rehabilitation, and along with Thomas Jefferson University Hospital, is one of 16 federally designated Regional Spinal Cord
Injury Centers. Inpatient and outpatient services are structured to provide lifetime rehabilitation and wellness programs

alternatives to providing services and ensure satisfaction.
Admission Process
Patients can be admitted to a Magee program directly from an acute care hospital, sub-acute facility or nursing home. Patients
must be at least 14 years old. Magee admits patients seven days a week. The process begins with a referral to the Magee Admissions
Department (215-587-3117). A referral may come from a patient’s attending physician, primary care physician, insurance company
representative, or hospital discharge planner (e.g. social worker, nurse, or case manager).
Once a referral is made, a Magee Clinical Liaison (a clinical evaluator) will visit the patient and family to discuss rehabilitation
options and to perform a clinical evaluation. During this evaluation, the Magee liaison can help patients, families and acute
care treating professionals to determine if rehabilitation is appropriate and the options available. Liaisons can also help
address insurance concerns.
Accreditation
Magee is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and CARF, The Rehabilitation
Accreditation Commission. Magee has been rated one of America's leading rehabilitation hospitals in U.S. News & World Report's
"America's Best Hospitals" rankings.
Tours of Magee
Prospective patients and families of prospective patients can schedule a tour of Magee through the Clinical Liaison or by
calling 215-587-3055. Tours of Magee are also available for groups, organizations and school classes to acquaint participants
with Magee and its services. For more information, contact the Marketing Department at 215-587-3055.
Parking
Parking is available during visiting hours in the Magee parking lot. There is no charge for immediate family. Several commercial
parking facilities are within easy walking distance. Magee is convenient to all forms of public transportation.

Individual circumstances may require flexibility with visiting hours. Special arrangements may be made with the assistance
of your Magee case manager. Children are welcome if accompanied and supervised by an adult.
Lodging
There are several hotels located near Magee. A listing of discounted rates at select hotels is available from case management
upon request.
Employee ID
All Magee employees wear identification badges. Feel free to ask any staff member for his or her name and job classification.
Smoking Policy
Your inpatient stay may be an excellent time to stop smoking. Patients at Magee may participate in a free smoking cessation
program while they are here. If interested, please discuss this with your case manager and physician.
Magee Rehabilitation Hospital is SMOKE FREE. Smoking is strictly prohibited inside the Hospital. Patients and visitors may
smoke outside the facility at designated smoking areas where ashtrays are located, away from the doors. In the interest of
your health and safety, and the health of others, please observe our SMOKE FREE policy.
PATIENT SERVICES
Phone and TV
To make your stay more pleasant, television and telephones are available for your use. The cost for phone service is one time
charge of $9.35 plus 7% sales tax (total amount $10) for local calls. Charges for long distance will be your responsibility.
There is no charge for television service, which includes cable access. The cost for TV service has been underwritten by a
charitable donation from the Friends of Jerry Segal.

Valuables
You may need a small amount of cash for personal items and telephone service. Snacks, toiletries and gifts may be purchased
at the Alcove gift shop, located in the main lobby. Please send all other money and valuables home. Patients are discouraged
from bringing personal electronic devices to Magee. Magee is NOT responsible for lost or stolen items.
Meals
Breakfast, lunch and dinner are served in the dining room on the second floor. You are asked to eat meals in the dining room,
unless otherwise instructed. Patients who have swallowing problems will eat their meals on the NeuroRehabilitation unit, or
in a special dining room (called the Dysphagia Dining Room). In order to decrease distractions to patients who are re-learning
how to swallow, visitors are not permitted in the Dysphagia Dining Room.
Meeting your nutrition needs is important to help you get stronger and healthier. Magee staff will help you make menu choices
from a special patient service line, will assist you in bringing food to your table, and will assist you in eating as needed.

Private Duty Nurses
Information concerning private duty nursing services is available through Case Management. Private duty nurses are not Hospital
employees. They work independently or for agencies. Fees for their services are paid directly to them or the agencies by
patients or their families.
Patient Safety
Magee is committed to safe and trusted healthcare. Partner with us to make your stay at Magee a safe one. Please request
and wait for assistance if you need to use the bathroom or get into or out of bed or wheelchair. If an unexpected event occurs,
we will notify your family as well as the State's Patient Safety Authority.
Gift Shop
Our gift shop is available to the public Monday through Saturday, 10:00 A.M. to 4:00 P.M. Volunteers and members of the Auxiliary
operate the gift shop. All proceeds are used to support the programs and services of the hospital.
Newspapers
Newspapers are delivered to patient floors daily
Mail
Mail is delivered to each nursing unit Monday through Friday, excluding holidays. Letters arriving after your discharge will
be forwarded to you home address. You can give outgoing mail to a member of the nursing staff.
Financial
Magee accepts most insurance plans. On the day of admission please have your insurance cards available to help the admission
process. When possible, we bill your insurance company directly. You are responsible for all charges not covered by your insurance
and will be billed for any outstanding balance.
YOUR HOSPITAL TEAM
Medical
The Physician is responsible for directing your care while you are a patient in the Hospital. As the coordinator for your
treatment program, your physician should be consulted if you have questions about your condition.
Case Management
Planning for your discharge begins at the point of admission. A case manager will be assigned, and will coordinate the services
you need prior to your discharge from Magee. They will consult with you and your physician about the need for continued services
and/or equipment when you leave Magee and will coordinate a safe discharge to home.
Nursing
A team of registered nurses, licensed practical nurses and nursing assistants provide nursing care around the clock. The
Nurse Manager is responsible for coordination of nursing care on each unit. Please feel free to contact your nurse or the
Nurse Manager on your unit if you have any questions or concerns regarding your stay at Magee. A Nurse Supervisor is available
on evenings and nights and the weekends to answer any questions or concerns you may have.
Occupational Therapy
An Occupational Therapist will be involved in your care if you need assistance with everyday tasks such as eating, bathing
and dressing.
Physical Therapy
A Physical Therapist will evaluate your strength, mobility and flexibility. The therapist will prepare an exercise program
to help you increase your strength and overall level of activity.
Recreational Therapy
A Recreational Therapist may lead group activities that promote self-expression and social interaction among group members.
Recreational therapy will also encourage increased activity through leisure and community activities.
Speech and Language
A Speech Therapist may meet with you to see how well you are swallowing food and liquids. If you are having problems with
speaking, understanding, reading or writing, the therapist will prepare a plan to assist you.
Psychology
The Psychology Department consists of licensed PhD level staff with considerable expertise in working with disabled and geriatric
populations. Most have specialty training and experience in assessing and treating cognitive deficits found in brain injury,
stroke, or neurological diseases such as Multiple Sclerosis and Parkinson's Disease.
Nutrition
The Hospital has registered dietitians to assist in meeting your nutritional needs during your stay. Please notify your nurse
if you have questions about your meals or diet so that the nurse may follow up with the dietitian.
Support Services
Providing quality care to each patient requires the diligent efforts of employees and staff throughout the Hospital. Some
of the employees you may meet directly include the Housekeeping staff, who are responsible for keeping your room clean, and
the Food Services staff. Other departments such as Admissions, Engineering and Maintenance, Laboratory, Medical Records,
Pharmacy and Radiology work behind the scenes to promote your comfort. Please notify your nurse if you have questions or
concerns about any of the Hospital services.
Volunteers
Our adult volunteers contribute many hours of service, supplementing our Hospital staff and adding a dimension of warmth and
caring to Magee.
YOUR ROOM
Room Assignments
Rooms are assigned depending on availability, patient’s medical needs and the patient population. At an additional charge,
private rooms may be requested, but may not be available due to medical necessity. Because of changes in hospital census,
occasionally patient rooms need to be changed.
Nurse Call Button
If you need to talk to your nurse at any time, press the call button located by your bedside. If you are unable to press
the call button, other devices are available. For your safety, there is also an emergency call button in the bathroom.
Fire Drills
At Magee we take fire safety seriously. That is why our staff receives ongoing training in fire safety and in responding
to other emergency situations. For your safety, the hospital regularly conducts fire and disaster drills. You will be notified
if a drill occurs during your hospital stay.
Staff Areas
Areas in the hospital that are designated as Authorized Personnel Only are restricted to Magee staff.
While You’re Here
Clothing and Laundry
Please have family and friends bring in your favorite comfortable clothes and shoes from home. We ask that your family do
all of your personal laundry and provide you with a clean supply of clothes. We suggest:
- Loose fitting clothing that is easy to put on and take off such as sweat pants
- Shirts or blouses and a sweater
- Undergarments
- Socks and rubber soled shoes or sneakers
- Seasonal outer wear for therapy outside of the building
Personal Items
You will need to bring a supply of personal toilet articles including shampoo, deodorant, dental care products, cosmetics
and shaving supplies.
Barber and Beauty Services
Area barbers and beauticians come to Magee for the convenience of patients. Appointments may be scheduled through the secretary
at the nursing unit.
Library
Magee’s medical and patient library, open to visitors, is located on the 3rd floor. The library is open during the week and
on weekends, and has computers with internet access for patient use.
Legal Clinic for the Disabled
The Legal Clinic is a non-profit organization housed within Magee. Its mission is to provide civil law legal services to
financially eligible disabled clients at no charge. At their “Open House”, scheduled every month, there is an opportunity
for Magee patients and family members to ask legal questions in an informal setting.
hireAbility
Housed in Magee, hireAbility is the regions leading employment agency dedicated to the service of people with disabilities.
hireAbility’s mission is to work with people with disabilities as they access the job market. hireAbility’s services are
free to both employers and applicants.
Pastoral Care Service
Pastoral care services are available at Magee by calling the Chaplain at 215-587-3092, or by visiting the Chapel on the fourth
floor. Persons of all faiths are welcome.
Interpreters
Interpreters are available to our patients. Your Case Manager can arrange these services for you. We also provide sign language
interpreters on request. Please let the Admissions Office or your Case Manager know as soon as possible if you will need
these services.
Drugs and Alcohol
Recreational drugs and alcohol are absolutely prohibited and may conflict with your medical management. Use of these substances
by patients or visitors will result in discharge from Magee.
GOING HOME
When You Leave
You and your family will be notified about the day and time of your hospital discharge. You should not leave the hospital
before being formally discharged by your physician.
The Magee team will work closely with you and your family to set up an individualized discharge plan. Discharge time is 9:00
a.m. Please discuss with your case manager if you will require a later discharge time.
On your discharge day, you will be taken to the main lobby exit. Any charges not covered by your insurance---such as the
additional cost for a private room or telephone---are payable at the time of discharge or you can be billed.
Follow-up
Following inpatient treatment your options may include outpatient therapy, Day Rehab or follow-up appointments with your Magee
physician. An outpatient case manager will be available to answer any questions you may have and notify you of all follow-up
appointments after discharge.
Patient Surveys
At Magee, your opinion counts. After your stay, you will receive a satisfaction survey in the mail at home. Please take
a few minutes to complete and return the survey. We use your feedback to recognize employees for a job well done and to improve
our services for our patients. We thank you in advance for sharing your thoughts with us.
You may also be telephoned 3-6 months after your discharge so that we may ask you about how you have been doing since you
left the hospital. Although this information is very helpful to us, you are under no obligation to provide it.
Support Groups
Magee Rehabilitation Hospital offers a variety of support groups for patients and their families. For more information call

Foundation
The Magee Rehabilitation Foundation exists solely to raise, manage, and disburse privately donated funds for the benefit of
Magee Rehabilitation Hospital and those we serve. As a non-profit, tax-exempt corporation, the Foundation relies on charitable
gifts from individuals, corporations, civic groups and private foundations to maintain and expand the hospital’s technology,
services and programs. If you would like to make a donation, the Foundation accepts gifts of cash, securities, life insurance
and mutual funds. You may also name Magee Rehabilitation Hospital as a beneficiary in your will or other estate plan. If
you would like to make a charitable gift, please contact the Foundation office at (215) 587-3090.
Hospitals Donations
Occasionally, patients or patients’ families desire to show their appreciation of special care that was given at Magee. If
you would like to acknowledge some member of the staff who made your stay comfortable you can mention them in your hospital
questionnaire. The accepting of money or gifts by individual employees from patients families, clients or from a person with
whom the hospital does business, is not permitted. Anyone wishing to make a donation or gift to the hospital can contact
the Development Office at (215) 587-3090.
MAGEE PROGRAMS AND SERVICES
The Spinal Cord Injury Rehabilitation Program
Since 1978 Magee, along with Thomas Jefferson University Hospital, forms the Regional Spinal Cord Injury Center of Delaware
Valley. This federally designated center is one of 16 in the country to provide coordinated lifetime care for people with
SCI and their families.
With more than 3,000 SCI patients in its system, Magee has the clinical experience and the unique peer resources that no other
regional rehabilitation program can offer.
The NeuroRehabilitation Center for Brain Injury
Magee Rehabilitation’s brain injury center of excellence was the first brain injury rehabilitation program in the nation accredited
by CARF, and in partnership with MossRehab, Magee is part of a model system for brain injury rehabilitation. The program is
built on years of experience in helping persons with brain injury recover to their maximum level of independence. The brain
injury care system at Magee uses proven, hands-on evaluations and therapies complemented by the latest technology.
The NeuroRehabilitation Center for Stroke
Magee’s Stroke Center is designed to treat the complex consequences of stroke. Treatment involves an organized effort to help
stroke survivors maximize all opportunities for returning to an active and productive lifestyle. Magee offers complete inpatient
and outpatient stroke rehabilitation programs.
The Orthopedic Rehabilitation Program
As orthopedic medicine has advanced, Magee has advanced the science of orthopedic rehabilitation. Today’s Orthopedic Rehabilitation
Program at Magee provides comprehensive rehabilitation to treat the loss of functional ability in patients with a wide variety
of conditions that affect muscles, bones and connective tissues.
The Amputation Program
The Amputation Program at Magee Rehabilitation is designed to assist people who have had an amputation adapt physically and
emotionally to necessary lifestyle changes. Rehabilitation goals are tailored to each patient and focus on increasing function
and independence in daily life activities. This may be an inpatient or outpatient program.
The General Rehabilitation Program
The General Rehab Program at Magee provides comprehensive rehabilitation to individuals with a wide variety of conditions,
such as arthritis, cancer, cardiac disease, Multiple Sclerosis, Parkinson’s Disease, neurologic disorders, Guillain-Barre
Syndrome, decline after a long medical illness, and after an organ transplant
The Ventilator Program
The Ventilator Program is a comprehensive program, which includes the latest in technology, patient education and case management.
Ventilator services are designed to increase the independence of patients whose breathing requires mechanical assistance as
a result of a spinal cord injury, neurologic disorder or other medical condition. Magee is one of only a handful of centers
in the country that can provide this specialized rehabilitation environment.
Outpatient Services
Magee’s outpatient programs are different from those offered by other healthcare providers because of the coordinated comprehensive
resources available to patients. These resources allow Magee to tailor programs to meet an individual patient’s needs. Outpatient
services range from formal day rehab programs to individual therapies.
Pain Management
Pain is an uncomfortable feeling that tells you something may be wrong in your body. Sometimes pain may be just a nuisance.
At other times, pain that doesn’t go away even after you take your pain medication may be a signal that there is a problem.
When you are admitted to the hospital you will be asked about pain. You will be asked to rate your experience of pain on
a scale of 0-10 (0 meaning you have no pain and 10 meaning very severe pain). It is important for you to inform your healthcare
provider if you are having pain, the severity of the pain and what have you done in the past that makes the pain either better
or worse. This information will help your doctor to find the best way to treat YOUR pain. Remember everyone’s experience
of pain is different. Only YOU can be the judge. There are a variety of methods available to control pain today and it is
important that you let your healthcare provider know if the type of pain control you are receiving is not working for you.
Reporting your pain is an important step in your recovery. Please discuss any concerns you have regarding your pain control
with your healthcare provider.
GRIEVANCE PROCEDURE FOR PATIENTS/FAMILY
Magee Rehabilitation Hospital, being committed to the highest possible level of care, has developed the following grievance
procedure for patients and family/guardians (when appropriate).
Every individual admitted as an inpatient or registered in Outpatient Services and their family/guardian (when appropriate)
has the right to file a medical (treatment related) or non-medical grievance with Magee Rehabilitation Hospital. To file
a grievance, the attending Physician, member of the treatment team, or the Executive Vice President and Chief Operation Officer
should be notified in writing or verbally.
An appropriate employee of the hospital will assume responsibility for resolving the grievance. This individual has the responsibility
for maintaining contact with the involved parties and informing them in writing about any decision made or actions to be taken
by the hospital. The grievance will be resolved in as a timely fashion as possible.
If for any reason the grievance is not resolved to the involved party’s satisfaction, the decision made by the hospital may
be appealed. Appeals should be directed to the Executive Vice President and Chief Operating Office of Magee Rehabilitation
Hospital. Appeal decisions will be made in as timely a fashion as possible.
If a patient feels that their rights have not been implemented by the hospital, they should notify the Chairman of the Board
of Trustees, and/or notify the Pennsylvania Department of Health by calling 1-877 PA-HEALTH.
INFORMATION THE FEDERAL GOVERNMENT ASKS THAT WE SHARE WITH YOU: MEDICARE
Patient Assessment
The Magee treatment team will assess your functional level within the first three days of your stay so that they may complete
the Inpatient Rehabilitation Facility Patient Assessment Instrument form. The Assessment is repeated towards the end of your
stay to measure your progress. Magee is required to provide the information in the Assessment to Medicare. As a Medicare
beneficiary you have the following rights:
The right to be informed of the purpose of the patient assessment data collection;
The right to have the patient assessment information collected kept confidential and secure;
The right to be informed that the patient assessment information will not be disclosed to others, except for legitimate purposes
allowed by the Federal Privacy Act and Federal and State regulations;
The right to refuse to answer patient assessment questions; and
The right to see, review, and request changes on the patient assessment instrument.
Your Rights While a Medicare Hospital Patient
You have a right to receive all of the care that is necessary for the proper diagnosis and treatment of your illness injury.
Your discharge date should be determined solely by your medical needs, not by Medicare payments. You have the right to be
fully informed about decisions affecting your Medicare coverage or payment for your hospital stay. You have the right to appeal
any written notices you receive from your hospital or Medicare stating that Medicare will no longer pay for your care.
Talk To Your Doctor
You and your doctor know more about your condition and your health needs than anyone else does. If you have questions about
your medical treatment, your need for continued hospital care or your discharge date, consult your doctor. These decisions
should be made between you and your doctor. If you have questions or concerns about hospital services, you should talk to
your Case Manager. Don’t hesitate to ask questions.
Peer Review Organizations
Peer Review Organizations (PROs) are staffs of doctors and nurses who are paid by the federal government to review medical
necessity, appropriateness, and quality of hospital treatment furnished to Medicare patients. Peer Review Organizations will
respond to your request for review and appeal of written Notices stating that Medicare will no longer pay for your hospital
stay.
If You Think You Are Being Asked to Leave the Hospital Too Soon.
Ask a Hospital representative for a written notice of explanation immediately if you have not already received one. The hospital’s
written notice is necessary if you decide to appeal to the PRO. The written notice must describe how you appeal.
If you decide to appeal, do so immediately by either calling or writing the PRO. If you lose the appeal, you may be billed
for all costs of stay beginning with the third day after you receive the written notice. The appeals process can take up
to three working days from the time your appeal request is received. Thus, if you appeal and lose, you may have to pay for
at least one day of hospital cost.
Remember: Without a written notice, you have no right to appeal.
Notice To Medicare Beneficiaries
In accordance with new federal requirements, the medical records of Medicare beneficiaries admitted to this hospital may be
subject to review by physicians and nurses employed by the Peer Review Organizations of Pennsylvania or its sub-contracting
agencies.
Major purposes of this review, which are done independently of this hospital, will be to determine both the need of medical
care and whether the hospital services rendered to you by your physician could have safely been administered at a less costly
alternate facility, such as a skilled nursing facility. Under the current regulations, if it is determined that your hospital
services could have been safely rendered outside the hospital, Medicare may not reimburse the hospital for all or part of
the services you receive. In the event of such determination, you, your physician, and the hospital will have the right to
appeal this decision.
Any information obtained from your hospital chart will remain confidential, subject to federal statute.
Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
Please review it carefully.
Who We Are
This notice describes the privacy practices of Magee Rehabilitation Hospital.
Magee facilities include all patient care, research, laboratory and administrative space owned or leased by laboratory and
administrative space owned or leased by Magee and any location where Magee staff work. All staff, students and other members
of the Magee community (“we or us”) follow the terms of this Notice. Magee is required by law to maintain the privacy of your
health information (“Protected Health Information” or PHI) and to provide you with Notice.
How We May Use and Disclose Health Information - Treatment, Payment and Health Care Operations
Except in an emergency or other special circumstance, we will ask you to sign a general authorization, as required by Pennsylvania
law, so that we may use and is disclose your PHI for the purposes detailed below:
Treatment
We may use and disclose your PHI connection with your treatment and/ or other services provided to you—for example, to diagnose
and treat you. In addition, we may contact you to provide appointment reminders or information about treatment alternatives
or other health-related benefits and services. We may disclose PHI to other providers (e.g. Physicians, nurses, pharmacists
and other heath care facilities involved in your treatment).
Payment
We may use and disclose your PHI to obtain payment for services that we provide to you—for example, to request payment from
your health insurer and to verify that your health insurer will pay your health care services.
Healthcare Operations
We use and disclose your PHI for our health care operations. These include internal administration and planning, and various
activities that improve the quality and cost effectiveness of health care services. For example we may use your PHI to evaluate
the quality and competence of our physicians, nurses and other health care workers. We may also use PHI to resolve patient
problems and complaints.
Other Healthcare Providers
We may also disclose PHI to other health care providers when such PHI is required for them to treat you, receive payment for
services they render to you, or conduct certain health care operations, for example, for emergency ambulance companies to
request payment for services in bringing you to the hospital.
Other Uses and Disclosures of Your PHI for Which Your Written Authorization Is Not Required
Use of Disclosure for our In-Patient Directory
If you are admitted to Magee, we may include your name, room number, general health condition and religious affiliation in
our hospital patient directory without obtaining your written authorization unless you object after reading this Notice. Information
in the hospital directory (other than religious affiliation) may be disclosed to anyone who asks for you by name, either in
person or by telephone. This information, (including your religious affiliation), may also be disclosed to members for the
clergy.
Disclosure to Relatives, Friends and Other Caregivers
You will be given a four (4) digit number when you are admitted (Patient Confidentiality PIN #). You must give this number
to family members, relatives, close personal friends, or any other persons that you identify, in order for them to receive
information related to your care. If people request information without the correct PIN number, Magee will not provide them
with your information
If we provide information to any individuals, we will release only information that we believe is directly relevant to the
person’s involvement with your health care or payment related to your health care. We may also disclose your PHI in the event
of an emergency or to notify (or assist in notifying) such persons of your location, general condition or death.
Fundraising Communications
We may contact you to request a donation to support important activities of Magee. We may disclose to our fundraising staff
non-medical information about you (e.g. your name, address and telephone number) and dates on which we provided health care
to you.
Public Health Activities
We may disclose your PHI for the following public health activities:
preventing or controlling disease, injury or disability;
reporting abuse and neglect to public health or other government authorities authorized by law to receive such reports.
reporting of deaths
reporting information about products and services under the jurisdiction of the United States Food and Drug Administration,
such as reactions to medications and problems with products;
alerting a person who may have been exposed to an infectious disease or may be at risk of contacting or spreading a disease
or condition;
notifying people of recalls of products they may be using; and
reporting information to your employer as required by laws addressing work-related illness and injuries or workplace medical
surveillance.
Victims of Abuse, Neglect or Domestic Violence
If we reasonably believe you are a victim of abuse, neglect or domestic violence, we may disclose your PHI to a governmental
authority, including a social service or protective services agency, authorized by law to receive reports of such abuse, neglect
or domestic violence.
Health Oversight Activities
We may disclose your PHI to health oversight agency that is responsible for ensuring compliance with rules of government health
programs such as Medicare or Medicaid.
Legal Proceeding and Law Enforcement
We may disclose your PHI in response to a court order, subpoena or other lawful process.
Deceased Persons
We may disclose PHI of deceased individuals to a coroner or medical examiner authorized by law to receive such information.
Obtaining Organs and Tissues
We may disclose your PHI to organizations that obtain organs or tissues for banking and/or transplantation.
Research
When conducting research, in most cases, we will ask for your written authorization before PHI is used. However, we may use
or disclose your PHI without your specific authorization if Magee’s Institutional Review Board (“IRB”) has waived the authorization
requirement. The IRB is a committee that oversees and approves research involving people.
Public Safety
We may use or disclose your PHI to prevent or lessen a serious and imminent threat to the safety of a person or the public.
Specialized Government Functions
We may release your PHI to units of the government with special functions, such as the U.S. military or the U.S. Department
of State under certain circumstances, such as for intelligence, counter-intelligence or national security activities.
Workers Compensation
We may disclose your PHI as authorized by state law relating to workers compensation or other similar government programs.
Inmates
If you are or become an inmate of a correctional institution or you are in the custody of a law enforcement official, we may
release your PHI to the institution or official if required to provide you with healthcare or to protect the health and safety
of others.
Patients Who Are Readmitted to Magee
If you are readmitted to Magee, staff members who you knew from a previous admission, but not part of your current treatment
team, may want to call on you. If you do not want this to happen, please notify us when you are admitted, and we will respect
your wish for privacy.
As Required By Law
We may use and disclose your PHI when required to do so by any other laws not already referenced above.
Uses and Disclosures Requiring Your Specific Written Authorization
For any purpose other the ones described above, we may use or disclose your PHI only when you give Magee your specific written
authorization. For instance, you will need to sign an authorization form before we send your PHI to a life insurance company.
The following are examples of other uses or disclosures for which your specific written authorization is required.
Marketing
We may use your PHI to communicate with you about products or services relating to your treatment, case management or care
coordination, or alternative treatments, therapies, providers or care settings without your written authorization. However,
we will obtain your written authorization prior to using your PHI to send you any other marketing materials.
Highly Confidential Information
Federal and state laws require special privacy protections for certain highly confidential information about you. This includes
PHI:

maintained in psychotherapy notes;
-
documenting mental health and developmental disabilities services;
-
about drug and alcohol abuse, prevention, treatment and referral;
-
relating to HIV/AIDS testing, diagnosis or treatment and other sexually transmitted diseases; and
genetic testing
Generally, we must obtain your written authorization to release this type of information. However, there are limited circumstances
under the law when this information may be released without your consent. For example, certain sexually transmitted diseases
must be reported to the Department of Health.
Your Rights Regarding Your Protected Health Information
Right to Inspect and Copy Your Health Information
You may request to see and receive copies of your medical and billing records. To do so please submit a written request to
the appropriate Magee office or department. You will be charged for copies in accordance with Pennsylvania law. If you are
a parent or legal guardian of a minor, certain portions of the minor’s medical record may be in accessible to you (for example,
records relating to abortion, contraception and/or family planning services) unless the patient authorizes Magee to give you
access to PHI. Additionally, under limited circumstances defined by law, we deny you access to a portion of your records.
Right to Request Restrictions
You may request additional restrictions on Magee’s use and disclose your PHI:

for treatment, payment and health care operation,
-
to individuals (such as family members or other relatives, close friends or any other person identified by you) involved with
your care or with payment related to your care, and
-
to notify or assist in the notification of such individuals regarding your location in the hospital and your general condition.
While we will consider all requests for restrictions carefully, we are not required to agree to a request.
Right to Receive Confidential Communications
You may request, and we will accommodate, any reasonable written request from you to receive your PHI by alternative means
of communication or at alternative locations. For example, you may instruct us not to contact you by telephone at home, you
may give us a mailing address other than your home for test results.
Right to Revoke your Authorization
You may revoke your authorization, except to the extent that we have already used or disclosed your PHI. A revocation form
is available upon request fro the Privacy Officer. This form must be completed by you and returned to the Privacy Officer.
Right to Amend your Records
You have the right to request that we amend PHI maintained in your medical or billing records. To do so, you must submit a
written request to the appropriate Magee office or department. We may deny your request if Magee reasonably believes that
the information is accurate and complete, if the PHI was not created by Magee, or other special circumstances apply.

If you desire further information about your privacy rights, are concerned that your privacy rights were violated, or disagree
with a decision that we made about access to PHI, you may contact our Privacy Officers at:

Right to Change Terms of This Notice
We may change the terms of this Notice at any time. If we change this Notice, we will post the revised Notice in appropriate
locations around Magee and on-line at www.mageerehab.org. You also may obtain any revised notice by contacting the Privacy Officer.
