Magee offers Health and Welfare plans that give employees a peace of mind by names they can trust; Independence Blue Cross, OptumRX, Inc., Delta Dental of Pennsylvania,Aetna DMO, and Vision Benefits of America. Our benefit program is comprehensive, flexible and cost effective and includes two choices of medical plans, dental plans, vision plans, a prescription plan and a variety of voluntary benefits.The employee benefits program is designed to provide you with a benefits package that is robust in obtaining better health and allows the employee to choose the best and most effective benefit plan to meet the needs of our employees. 

Medical Plans

Magee Rehabilitation medical plan options are offered through Independence Blue Cross (IBC) and not only covers preventive care services and hospital stays, but health value-added programs to lose weight, quit smoking, and manage stress. Magee’s plans are the right plans that motivates employee engagement through getting healthy and staying healthy.

You can choose from the following options:

  • Keystone Health Plan East (HMO)
  • Personal Choice (PPO)
  • Waive Coverage
  • Keystone Health Plan East (HMO)

Health Maintenance Organizations (HMO) requires an individual to select a primary care physician (PCP) from the Keystone network, who coordinates your care. If you do not use a PCP or use providers within your plans network, benefits will not be paid. There are no annual deductibles or claim forms to file.

This plan offers two tiers of coverage to accommodate your family’s needs.

You can receive care from:

  • Tier I – Delaware Valley Accountable Care Organization (DV-ACO) providers (i.e. Magee, TJU, MLH, Methodist, and AI DuPont) who participates in the ACOPlus Referred Care Keystone network. Non-JHS ACO Plus Facility (i.e Virtua) for inpatient only care.
  • Tier II – Keystone HMO Referred Care in network facilities and providers
    Personal Choice (PPO)

Preferred Provider Organizations (PPO) does not require you to select a PCP or obtain referrals to see specialists. You have a choice of three tiers of coverage when you select Personal Choice.

  • Tier I – JeffPLUS JHS facilities and providers (i.e. Magee, TJU, MLH and Methodist who participates in the Personal Choice network. JeffPLUS Non-JHS facilities and providers (i.e. Nemours and Virtua)
  • Tier II – Personal Choice (PPO) in network facilities and providers (i.e. Abington, CHOP, etc.). You are required to pay deductibles of $500 or $1,000 for either individual or family services respectively.

Personal Choice (PPO) Out-of-Network does require deductibles of $1,000 or $2,000 for either individual or family services respectively. Office visits are covered at 70% after deductible with an unlimited lifetime maximum.

OptumRX* administers Magee Rehabilitation prescription pharmacy benefits and employees may use any retail pharmacy.  Employee may also reduce their out-of-pocket expense by using Jefferson Outpatient Pharmacies.  Magee Rehabilitation has implemented a 4-tier formulary co-payment plan.  The retail co-payments for a 30 day supply are shown and co-payments when employees use Jefferson Pharmacy.

You are required to purchase generic drugs when available. Otherwise, you will berequired to pay the different in cost between the generic and the brand name and-applicable co-pay. The 2015 out-of-pocket limits are $2,500 per person or $5,000 per family

Jefferson Pharmacy Advantage allows an employee to purchase prescription    drugs at the Jefferson Outpatient Pharmacy.  Your retail co-pay per prescription will be $15.00 for generic and $30.00 for brand name drug, for a 34-day supply.  The Jefferson Outpatient Pharmacies offers you the option of purchasing a 90-day supply of certain medications. The co-pay will be $20.00 for generic and $30.00 for Brand/Formulary.  Jefferson Outpatient Pharmacies does not have a third tier drug option as a Brand/Non-Formulary drug and a fourth tier Specialty drugs. 

Management Program

MRH implemented four prescription management programs to improve care and help manage costs for the plan members and the organization. The four prescription management programs are: Prior Authorization, Quantity Limits, Step Therapy, and Compounding Medication.

Magee’s Employee Pharmacy

Magee’s pharmacy benefit is administered in-house and retail. As an added advantage, you as an employee can purchase prescription drugs at the Jefferson Outpatient Pharmacies in 5 locations and a Drop- off/Pick-up on site at your fingertips.

Dental Plans

You can choose from the following options:

Low Plan

The Low option, offers you the freedom to receive dental care from any licensed dentist you choose for a variety of preventive, restorative and specialty care services. When selecting a dentist in Delta Dental Network, theannual maximum per person is $1,700. Selecting a Non-Delta Dentist the annual maximum per person is$1,500. High Plan

The High Plan option, offers you the freedom to receive dental care from any licensed dentist you choose for a variety of preventive, restorative and specialty care services. When selecting a dentist in Delta Dental Network, theannual maximum per person is $2,000. Selecting a Non-Delta Dentist the annual maximum per person is$1,700. Aetna DMO

Aetna DMO is a managed care dental organization which requires the employee to select a primary care dentist to provide routine dental care, some specialty treatment and refer you to a specialist when necessary. There is no deductible and no claims forms to process. Services are not covered when care is received from an out-of–network provider.

Aetna DMO, will cover 100% payment of preventive and basic restorative care including routine examinations, cleaning, and partial x-rays every six months and full mouth x-rays every 36 months. Major Restorative care is covered at 50% and Orthodontic care with no lifetime maximum. There is no annual deductible. A list of all participating dentists may be obtained by accessing www.aetna.com. Vision Plans

You can choose from the following options:

  • Keystone HMO (only for participants in the HMO benefit)
  • Vision Benefits of America Core Option Plan (VBA)
  • Vision Benefits of America Plus Option Plan (VBA)

The Keystone HMO vision rider program is administered by Davis Vision which offers members corrective eyewear, including eyeglasses or contact lens. You may receive a free eye-examination once every two calendar years. Members will receive allowances up to $35.00 for participating providers for corrective eyewear. Please go to www.ibx.com to locate a participating “Vision Provider”.

Vision Benefits of America Standard and High Option Plans (VBA)

VBA represents one of most comprehensive of Optometrists, Ophthalmologist and well known Retail Optical Stores in the nation. The plans provide complete vision exam, clear lenses and a quality frame at no-out-pocket cost to you.

You may search for providers by visiting the website at www.visionbenefts.com or call and speak to the Customer Service Department at (1-800-432-4966). You doctor can obtain a benefit form via e-mail and it is sent directly to you provider

Life Insurance

The Life insurance plan is administered through Reliance Standard Life Insurance.

Employees are covered for one (1X) their annual base salary. This coverage also includes an Accidental Death and Dismemberment clause related to the principal amount of the insurance.

The life insurance benefit covers all fulltime and part-time employees (who are budgeted to work at least 20 hours weekly. This is an employer paid benefit.

Long Term Disability

The Long Term Disability plan is administered through Reliance Standard Life Insurance. The Long Term Disability plan is designed to provide a monthly payment equal to 50% of monthly salary up to $7,500 per month (less any social security benefits). An employee must be totally disabled for six months and approved by insurance company.

Short Term Disability

The Short-Term Disability plan is designed to partially replace income lost during periods of total disability from a covered injury or sickness resulting from a non-occupational cause.

Benefits start after 13 weeks of total disability and all sick time has been exhausted. The benefit is 60% of weekly salary with a maximum of $300 per week and will be paid from the 14th through the 26th week of absence.

Group Legal Plan

In an ongoing effort to provide greater benefits to you, Magee offers a voluntary group legal plan through Legal EASE Services.

This legal plan was designed to make legal services available and affordable to you when the need arise to provide you with personal legal protection for you and your family.

There are a number of legal services provided for you, your spouse and dependents such as:

  • Estate Planning
  • Preparation of Wills
  • Divorce
  • Child Custody

Other matters (ID Theft Assistance, Foreclosures, and Tax Audits, etc.) Any questions about the legal service plan can be answered by calling LegalEASE Services at (1-888-416-4313).

Employee Assistance Programs

The Employee Assistance Program (EAP) provider is Jefferson Health Systems

First Call

FirstCALL offers confidential and private consultation on a broad range of personal and emotional matters such as: marital, stress, drug/alcohol services, child and elder care.

Self-referral services are directly available to all employee and dependents on a 24-hour a day, 7 days-a week basis. The number is 1-800-382-2377.

Carebridge

Carebridge is a corporation that provides you help in successfully managing family and personal concerns which can negatively impact your work performance and quality of life. The counselors are trained to provide you the broadest set of options such as: child care, college planning, eldercare, adoption information, person financial management, continuing education, life balance issues and time management.

Carebridge provides access to assistance 24-hours a day, 7 days-a week basis. The number is (1-800-437-0911).

Pension Plans

Magee Rehabilitation Hospital provides eligible employees with a Defined Pension Plan and 403B ERISA Plans that will provide a retirement benefit to supplement social security benefits.

Defined Pension Benefit Plan

All employees who were hired before January 1, 2015 as fulltime or part- time are eligible to participate in the Defined Pension Plan that provides a retirement benefit and supplement social security benefits. An employee who works at least 1,000 hours per year are eligible to be enrolled. The vesting period is five years of service and the benefit is based on the employee’s salary history, years of service, and age. The normal retirement benefit is payable at age 65, however, a reduced pension prior to age 65 may be possible based on eligibly. Magee Rehabilitation contributes the full cost of the pension plan. More information is available in the Summary Plan Description located on Magee’s Intranet.

ERISA 403B

All employees who are hired on or after January 1, 2015 can join the 403B ERISA Plans, first day of the month after an initial 30 days of employment. MRH will automatic enroll an employee in the plan selecting a 3% contribution withdraw bi-weekly from their check. The employee have a choice of increasing or decreasing that amount. The employee must work at least 1,000 per year to actively become a participant in the plan. Magee Rehabilitation will match an employee’s contribution on fiscal year up to 6%.

The 403B plan contributions are invested to provide benefits on your behalf and contributions are pre-tax electives to reduce your total taxable income which reduces your current taxes. An Employee Retirement Income Security Act (ERISA) provides rules about the investment of retirement funds.

Paid Holiday

Full time Employees

All regular, full-time employees are entitled to six legal holidays that include New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.

Part-time Employees

All regularly scheduled part-time employees are entitled to six legal holidays that include New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day. However, the amount of hours paid for these holidays are based on budgeted hours worked from 40, but less than 80 bi-weekly.

Paid Time Off (PTO)

Full time Employees
  • All full time employees are granted Paid Time Off (PTO) days depending on employee group and length of service.
  • All hourly, clerical and support staff are non-exempt Professional and management staff are exempted employees.
Part-time Employees
  • All part-time benefit eligible employees budgeted for at least 40 hours bi-weekly, but less than 80 hours will accrue Paid Time Off (PTO) on pro-rated bases.
  • A part-time employee will accrue PTO based on hours worked up to the maximum number of PTO days accrued by a full-time budgeted position. Paid Time of (PTO) days is granted depending employee group and length of service.

PTO will be used for: Vacation Time, Personal Time and Short-Term Sickness. The maximum allowable PTO bank will be one year equivalent for each person based on years of service.

Extended Illness Bank (EIB)

Full Time Employees
  • An Extended Illness Bank is granted to all full-time, regular employees as a benefit for payment when absent from work due to their own sickness or disability.
  • Eligible employees accrue 10 EIB days per year. EIB days are carried over from year to year with no limits. An employee must use 2 PTO days prior to accessing their EIB bank.
Part Time Employees
  • All part-time benefit eligible employees who are budgeted for at least 20 hours per week will accrue EIB days on pro-rated bases.
  • A part-time employee will accrue EIB days based on hours worked up to the maximum number of EIB days accrued by a full-time budgeted position.

Funeral Leave

Full Time Employees

All regular, full-time employees may be granted up to three (3) days off with pay for death of certain family members.

Part Time Employees

All part-time employees may be granted one (1) day of absence from schedule duty with pay in the event death of certain family members.

Jury Duty

All regular, full-time employees will receive their regular pay when the jury duty check has been signed over to Magee.

Tuition Assistance

Full Time Employees
  • Reimbursement is provided to full-time, regular employees for courses which are work-related or used within the organization.
  • Undergraduate students must complete the course with a grade of “C” or better. Maximum benefits for undergraduate courses are up to $2,500 per year.
  • Graduate students are compensated up to $5,000 per year for graduate courses. Graduate students must complete the course with a grade of “B” or better.
Part Time Employees
  • Reimbursement for part-time employees regularly scheduled to work 40 or more, but less than 80 hours bi-weekly are eligible to receive tuition reimbursement on a pro-rated bases, according to budgeted hours. Reimbursement is up to a maximum of two courses per semester.
  • Employees who receive tuition reimbursements are expected to remain in an eligible position for a minimum of 12 months after completing the course.

Credit Unions

This program enables employees to use the credit unions as an option for savings and provides for financial assistance such as loans. Deposits are available through each of the following credit unions:

National Business Association Credit Union (NBA), in Bristol, Pa

(SB1) Federal Credit Union, in Philadelphia, PA.

Wage Works

All full-time and part-time employees are eligible to participate in the commuter benefits tax saving plan through Wage Works.

Employees will be able to pay for the transit and parking cost through payroll deductions on a pre-tax basis. The allowable deduction for public transportation and parking is $220. The pass is mailed directly to your home on a monthly basis. Commuters who ride on SEPTA, PATCO, and New Jersey Transit can take advantage of this program.
