You can choose from the following options:

JeffPLUS Keystone HMO
JeffPLUS Personal Choice PPO
Waive Coverage

JeffPLUS Keystone HMO

This Health Maintenance Organization (HMO) is administered through Independence Blue Cross (IBC).

Through the JeffPLUS Keystone HMO option you have two tiers of coverage. You can receive care from:

JeffPLUS JHS facilities and providers (i.e. Magee, TJU, MLH and Methodist who participates in the Keystone network.

Keystone HMO in network facilities and providers (i.e. Abington, CHOP, etc.).

You must select a primary care physician (PCP) from the Keystone network, who coordinates your care.

If you do not use a PCP and use providers within your plans network, benefits will not be paid.

There are no annual deductibles or claim forms to file.

JeffPLUS Personal Choice PPO

This Preferred Provider Organization (PPO) is administered through Independence Blue Cross (IBC).

Through the JeffPLUS Personal Choice option you have a choice of three tiers of coverage. You can receive care from:

JeffPLUS JHS facilities and providers (i.e. Magee, TJU, MLH and Methodist who participates in the Personal Choice network.

JeffPLUS Non-JHS facilities and providers (i.e. Nemours and Virtua)

Personal Choice Җ (IBC) in network facilities and providers (i.e. Abington, CHOP, etc.).

You do not need to a PCP or obtain referrals to see specialists.

If you do not obtain services from a JeffPLUS provider, you will be responsible to meet an annual deductible before the plan pays benefits.
