The Magee Riverfront team of experts includes a rehabilitation nurse, physical and occupational therapists, speech/language pathologists, psychologists, biofeedback specialist, recreation therapists, vocational counselor, case manager, geriatric clinical specialist and ergonomic specialist.

Case Management

Case management services are designed to assist patients to navigate through the healthcare environment. Many issues outside of a patient's medical care can impact the patient's ability to maximize progress towards achieving independence.

Magee Riverfront has a dedicated case manager to assist patients to resolve those issues which may limit their ability to reach their goals. Examples of such issues include:

  • Transportation
  • Financial
  • Community referrals, e.g. home care
  • Re-entering the workforce
  • Social security and disability issues
  • Welfare
  • Couples therapy
  • Family counseling&education on disability and recovery
  • Referrals to senior centers/adult day care
  • Utilization review with insurance company and external case managers
  • Durable medical equipment

Referrals for case management assistance can be made by any of the Magee staff, or a patient or family member may directly ask for assistance.
